To qualify for Illness/Injury social welfare certification you must be seen by a GP initially. To extend a period of cover, you must complete this form.
To qualify for statutory sick pay from your employer you must be certified by a GP as unable to work.
Social welfare certs are free but private work certs must be pre-paid. Upon receiving your request, if applicable, we will text you a billink which you can click on to submit payment. Your GP may call you or request for you to be seen. If you need to be seen, you will only be charged the balance remaining for a standard consult.
